Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a burn/fall hazard:
At this inspection site, on May 17, 2007, and at times prior thereto, the
auxiliary
escape line was installed in an unsafe manner and included the following
deficiencies:
a.A securely anchored one-half inch wire rope escape line was attached to
derrick or mast so as not to provide a ready and convenient means of
egress from the Derrickman's working platform.
b.The Emergency Escape Device was installed on the wire rope escape line
which was secured to the center of a fabricated steel tube support above
the top guardrail on the east side of the pipe racking board.
c.The Emergency Escape Device upright bar was away from the pipe
racking board with the upside down "T" bar above the walking surface of
the pipe racking board.
d.The one-half inch wire rope escape line was stretched out.
e.The end of the wire rope escape line had been passed through a chain link
to form an eye with the dead end of the wire rope extending back beyond
and laying along side of the live wire rope line.  There was no thimble for
the eye.
f.Only two Crosby clips were used to secure the dead end of the wire rope
to the live end and they were installed backward, i.e. saddle on the dead
end and the "U" bolt on the live end.  There was no tail on the dead end
of the wire rope, i.e. the second Crosby clip fit right over the end of the
dead end.
