The employer did not furnish to each of his employees employment and a
place of employment
which were free from recognized hazards that were causing or were likely
to cause death or
serious physical harm to employees in that employees were exposed to the
hazard of falling:
On or about 06/19/2007, it was identified that the employer allowed the
improper use of the
equipment against the manufacturers safety recommendations.  Company
practice is to have
employees standing in the basket of a bucket truck while the boom was
being used for a lifting
operation. The employee was exposed to being propelled out of the bucket
from
the loss of the
integrity of the sling connection to the lamp post at 2000 NW 55th Avenue,
Lauderhill, Fl
33313.
Among other methods of abatement, one feasible and acceptable method to
correct this hazard
is to ensure that manufacturer's safety recommendations such as keeping
employees out of the
basket during lifting operations are followed.
PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS
REQUIRED FOR THIS VIOLATION.
