Section 5(A)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards: On or about 09/12/2007, an employee was exposed to the hazard of being struck and crushed by the load of a powered industrial truck that was being loaded beyond its capacity. Among other methods of abatement, one feasible and acceptable method to correct this hazard is to ensure that employees are prohibited to overload forklifts beyond its rated capacity as recommended by ANSI/ITSDF B56.6-2005. PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
