On or about 03/20/2008, at 8202 NW 98th St., in the city of Hialeah
Gardens, an employee was
exposed to the hazard of being struck and crushed by a Rough Terrain
Forklift Truck that was
lacking boom angle indicator.
Among other methods of abatement, one feasible and acceptable method to
correct the hazard
is to provide the equipment with a boom angle indicator visible to the
operator as recommended
by ANSI/ITSDF B56.6-2005.

REQUIRED FOR THIS VIOLATION.
