Section 5(A)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards: On or about 03/20/2008, at 8202 NW 98th St., in the city of Hialeah Gardens, an employee was exposed to the hazard of being struck and crushed by a Rough Terrain Forklift Truck that was lacking boom angle indicator. Among other methods of abatement, one feasible and acceptable method to correct the hazard is to provide the equipment with a boom angle indicator visible to the operator as recommended by ANSI/ITSDF B56.6-2005. PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
