On or about 04/09/2008, Building # 2, an employee was exposed to the
hazards of falling and
being crushed by a powered industrial truck while operating a forklift
without wearing the seat
belt.
Among other methods of abatements, one feasible and acceptable method to
correct this hazard
is to prohibit employees from operating this type of equipment without
wearing seat belts.
PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS
REQUIRED FOR THIS VIOLATION.
