Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to a struck-by hazard: a)Building A, Crane Area - A spreader bar that had been manufactured at the facility had not been proof tested, inspected or marked for its rated capacity prior to being put into service lifting truck boxes weighing up to 5800 pounds or more, on or about April 12, 2007. Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure the spreader bar is proof tested, inspected and marked in accordance with ASME B30.20-2006, Below-the-Hook Lifting Devices, Chapter 20-1.2.1, Chapter 20-1.3.1 and Chapter 20-1.3.8.1.ook

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to a struck-by hazard: a)Building A - The 8 ton monorail underhung crane that was used to install a truck box onto a chassis had not been inspected prior to its initial use in the facility lifting truck boxes weighing up to 5800 pounds or more, on or about April 20, 2007. Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure the crane is inspected in accordance with ANSI B30.11-2004, Monorails and Underhung Cranes, Chapter 11-2.1.1.

Among other methods, one feasible and acceptable abatement method to correct this hazard is to place a cover over the foot bar in accordane with ANSI B11.4-2003, Safety Requirements for Shears, Chapter 6.3.3.3.
