North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a) Facility, ATU line- Jib Crane with 1000 lbs capacity and one hoist was not load tested prior to initial use. REF: ANSI B30.11-1998, Chapter 11-2, Paragraph 11-2.2.2 (a)New, reinstalled, altered, repaired and modified equipment should be load tested prior to initial use as determined by a qualified person. (c)If a load test is conducted, the person conducting the load test shall prepare a written report of the load sustained during the test and the operations performed during the test. Reports shall be placed on file. NOTE: Among other methods, one feasible and acceptable method to correct this hazard is to load test the Jib Crane per the requirements of section 2.2.2 of ANSI B30-11-1998.

North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a) Facility, ATU line - Jib Crane with 1000 lbs capacity and one hoist was not inspected periodically. REF: ANSI B30.11-1998, Chapter 11-2, Paragraph 11-2.1.3 (a)Periodic inspections shall be performed at intervals defined in para. 11-2.1.1(b)(2): Visual inspection by an appointed person making records of apparent external conditions to provide the basis for a continuing evaluation, as follows: normal service, equipment in place --- annually. (c)A qualified person shall examine any deficiencies found during the inspection to determine whether conditions found constitute a hazard, require disassembly for further inspection, or require replacement. NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to conduct periodic inspections (annual) on the jib crane per the requirements of section 11-2.1.3 of ANSI B30-11-1998.

Visual examination by the operator or other designated personnel, with records not required, as follows: normal service ---- monthly. NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to conduct frequent inspections (monthly) on the jib crane per the requirements of section 11-2.1.2 of ANSI B30-11-1998.
