approximately nine 3/4 inch diameter
metal form stakes, protruding 4 to 10 inches above the bracing members of
the concrete forms where employees
were working/walking, were not guarded in any manner to eliminate the
hazard of impalement. Among other
methods, one feasible and acceptable abatement method to correct this
hazard is to use protective caps on the top
of the protruding metal form stakes.
