Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling to the ground from an extended scissor lift. a) Worksite - Two employees were operating and using an extended scissor lift (JLG Electric Scissor Narrow, Model 2630ES, Serial 0200133009) at a height of approximately 15 feet with a 100-feet extension cord tied to the rails while connected to an outlet on a wall and did not take special precautions when other moving equipment was present in the area; on or about 04/04/08. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19. ABATEMENT NOTE: Among other methods, feasible methods of abatement to correct this hazard include, but are not limited to: Follow the JLG Model 2630ES Operation and Safety Manual, section 1, page 1-4 (Safety Precautions, Tipping Hazards), which states: "Never attach wire, cable, or any similar items to platform."
