a) Jobsite on or about May 28, 2009 an employee was exposed to a fall
hazard while riding
in the bucket of a Komatsu PC 300 Excavator S/N A90143CC  taking the
measurements of
an excavation.  Among other methods feasible and acceptable means of
abatement would be
to follow the manufacturer's guidelines set forth in the operating
procedures manual Page 1-
10 that states:  Never let anyone ride on the bucket, grapple, clamshell
or other attachments.
There is a  hazard of falling and suffering serious injury.
Note:  In addition to abatement certification, the employer is required to
submit
abatement documentation for this item in accordance with 29 CFR 1903.19.
