were exposed to being caught by rotating machine parts:
a)Floor 5:  Employees who performed pipe threading were exposed to
contacting
nd being caught by the rotating parts of a Ridgid Model 300 Pipe Threading
Machine (s/n 700982) which was provided with a momentary contact foot
switch
which was not operable.  Employees caught by the rotating parts may be
unable
to reach the control switch located on the machine body in order to stop
the
machine before serious injury or death occurs.  On or about 7/3/07.
Abatement Note
Among others, a feasible and useful method to eliminate or to materially
reduce this
hazard would be to repair or replace the defective momentary contact foot
switch.
NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF
ABATEMENT FOR THIS ITEM.  FAILURE TO COMPLY MAY RESULT IN AN
ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.
