were causing or likely to cause death or serious physical harm to

were exposed to an amputation hazard:
a) Production Area - Where point of operation guarding was not feasible,
the employer
did not develop and implement a minimum safe distance program for the
protection of
employees who operated press brakes including, but not limited to 123 ton
Darley press
brake (model EHP 110 31/25, sn #12800) and the 60 ton Darley press brake
(model EHP
50 25/20, sn #122143). On or about 7/10/07.
One feasible and acceptable abatement method is to develop and implement a
minimum
safe distance program that conforms to the requirements of ANSI
B11.3-1982, Paragraph
6.1.4.3 and OSHA Directive CPL 2-1.25.
NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT
CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN
ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
