General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to a struck-by hazard: a) Blast room - Employees were using lifting devices, a spreader bar and a lifting lug, that were manufactured at the facility. The lifting devices were not proof tested, inspected or marked for their rated capacities prior to being put into service. The employees used the spreader bar to lift plate steel and the lifting lug to lift steel piles. The plate steel weighed approximately 2000 pounds and the steel piles weighed approximately 3200 pounds, on or about 6/20/07. Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure the spreader bar and lifting lug are proof tested, inspected and marked in accordance with ASME B30.20-2006, Below-the-Hook Lifting Devices, Chapter 20-1.2.1, Chapter 20-1.3.1 and Chapter 20-1.3.8.1
