Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to burn/fall hazard:
At this inspection site, on July 12, 2007, and at other times prior
thereto, the
auxiliary escape line was installed in an unsafe manner and included the
following
deficiencies:
a.A securely anchored one-half inch wire rope escape was attached to
derrick/mast so as not to provide a ready and convenient means of egress
from the Derrickman's working platform.
b.The Emergency Escape Device was installed on the wire rope escape line
with the wire rope escape line secured to the South leg of the derrick/mast
above the pipe racking board and the wire rope escape line was pulled
over to the top guardrail with a manila rope.
c.The wire rope escape line was not secured high enough on the South leg
of the derrick/mast to allow proper access to the Emergency Escape
Device, i.e. the wire rope escape line was below the top guardrail on the
South side.
d.The one-half inch wire rope escape line was stretched out, with too much
slack, i. e. the escape line was laying on the ground between the stake
anchor and the upright derrick/mast.
e.The end of the wire rope escape line had been placed in the grooved area
of a thimble to form an eye with the dead end of the wire rope extending
back beyond the thimble and laying along side of live end.  Only two
Crosby clips were used to secure the dead end of the wire rope to the live
end and they were installed backward, i.e. saddle on the dead end and the
"U" bolt on the live end.
f.The end of the wire rope escape line was improperly secured.  A chain
was passed through the thimble eye and then tied into a "Hog knot."  The
remainder of the chain then passed around the steel stake with the end of
the chain containing a hook and the hook hooked back into one of the
chain links.
One feasible and acceptable abatement method would include ensuring the
Auxiliary Escape is installed according to manufacturer's recommendations.
