Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish to each of his employees employment and a place of employment
which are free from
recognized hazards that are causing or are likely to cause death or
serious physical harm to
his employees in that employees were exposed to:
(a) On September 23, 2008 and times prior there to, employees were exposed
to struck by
hazards while over pressuring the relief manifolds following the
fracturing process.
Feasible and acceptable methods to correct this hazard would be to secure
the
manifold to the
well head by the use of chain or wire rope as per API document #54 section
9.13.3, and
operate piping and valves within there rated working pressures. Securing
the manifold
beyond hand tight by the use of tools such as a pipe wrench.
