(a) Drilling Rig:  Employees were exposed to the hazards associated with
wire failure while
hoisting drill pipe and other drill equipment due to the damaged wire rope
on the drum hoists
located above the driller controls.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is follow the requirements set forth in the ASME B30.7-2001, Section
7-2.4.1 (c) (2) which
sets forth the removal criteria wire rope for damage such as kinking,
crushing, bird caging,
or any other damage resulting in distortion of the rope structure.

being struck by the
derrick in the event of a tip over in that the guy wires stabilizing the
derrick were
inadequately secured with the use of only two u-bolt clips.
Among others, one feasible and acceptable method to correct this hazard
would be to follow
the API document, Practice 9B, 11th Edition, 2002, titled, "Recommended
Practice on
Application, Care and Use of Wire Rope for Oilfield Service," Section
3.7.4 which requires
the use of at least four wire rope clips when a sheave is in place.
