29 CFR 1910.119(h)(3)(ii):  The contract employer did not assure that each
employee was
instructed in the applicable provisions of the emergency action plan:
a.  On or about March 18, 2009 and at times prior ASI Piping did not
ensure that
employees were protected from explosion/fire/chemical exposure hazards.
The
employees were not properly instructed concerning provisions of CFR 1910.38
Emergency Action Plans.
1.  CFR 1910.38(c)(2):  Calls for types of evacuations and exit route
assignments.  The employees were not given clear instructions on types of
evacuations and specific techniques (wind direction and strength) to be
used in
making a safe escape.  They were also not given a clear exit route when
moved around the plant
2.  CFR 1910.38(d):  Calls for a distinctive signal for each purpose.  The
employees did not know how many signals or what each distinctive alarm
meant.

corridors, offices, shops or storage areas were not lighted to the minimum
illumination
intensities listed in table D3 of subpart D of 29 CFR part 1926:
a)  On or about March 18, 2009 and at times prior ASI Piping Inc. did not
ensure that
employees were protected from lack of lighting hazards.  Employees, doing
demolition
work with cut off tools, were not provided lighting behind the fire
blankets used to
contain sparks.

a)  On or about March 18, 2009 and at times prior ASI Piping Inc did not
ensure that
employees were protected from electrical hazards.  The electrical system
used for the
turnaround demolition work was not inspected to assure the system to be
free from
recognizable hazards.

for the hazardous
(classified) location or safe for the hazardous (classified) location:
a)  On or about March 18, 2009 and at times prior ASI Piping did not
ensure
that
employees were protected from electrical hazards.  The crew doing
demolition work on
the Coker de-heading deck did use electrical equipment which was not
approved for the
electrical classification of this area.  This included the tools, cords,
and supply pendant
used for the demolition crew.
