29 CFR 1910.119(h)(3)(ii):  The contract employer did not assure that each
employee was
instructed in the applicable provisions of the emergency action plan:
a.  On or about February 23, 2009 and at times prior Colstrip Electric Inc
did not
ensure that employees were protected from explosion/fire/chemical exposure
hazards.
The employees were not properly instructed concerning provisions of CFR
1910.38
Emergency Action Plans.
1.  CFR 1910.38(c)(2):  Calls for types of evacuations and exit route
assignments.  The employees were not given clear instructions on types of
evacuations and specific techniques (wind direction and strength) to be
used in
making a safe escape.  They were also not given a clear exit route when
moved around the plant
2.  CFR 1910.38(d):  Calls for a distinctive signal for each purpose.  The
employees did not know how many signals or what each distinctive alarm
meant.

equipment in hazardous
(classified) locations were not approved as intrinsically safe or approved
for
the hazardous
(classified) location or safe for the hazardous (classified) location:
a)  On or about March 17, 2009 and at times prior Colstrip Electric Inc.
as the
exposing/creating contractor, did not ensure that employees/workers were
protected from
electrical hazards.  The crew doing demolition work on the Coker
de-heading deck did
use electrical equipment which was not approved for the electrical
classification of this
area. This included the conduit bender, cords, and supply pendant used by
the demolition
crews.  The pendant was installed in the classified area by Colstrip
Electric Inc.
