(a) LG Contractors fabrication shop within the ExxonMobil Billings Refinery: On or about December 1, 2009 and at times prior thereto, LG Constructors, Inc., used three 115 volt flexible cords as a substitute for the fixed wiring of a structure. Employees were subject to shock and electrocution hazards.

Employees were subject to shock and electrocution hazards.

29 CFR 1910.305(a)(2)(x) Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points. (a) LG Contractors fabrication shop within the ExxonMobil Billings Refinery: On or about November 19, 2009 and at times prior thereto, LG Constructors, Inc., used flexible DC welding conductors from a Miller Mark VIII DC welder which were subject to damage due to the placement of a metal toolbox on top of the conductors. (b) LG Contractors fabrication shop within the ExxonMobil Billings Refinery: On or about November 19, 2009 and at times prior thereto, LG Constructors, Inc., used flexible DC welding conductors from a Miller Mark VIII DC welder which were subject to damage by the sharp corners of a hole made in the wall of the metal fabrication shop building.
