welding conductors from a Miller Mark VIII  DC welder which were subject
to damage due
to the placement of a metal toolbox on top of the conductors.
(b) LG Contractors fabrication shop within the ExxonMobil Billings
Refinery: On or about
November 19, 2009 and at times prior thereto, LG Constructors, Inc., used
flexible DC
welding conductors from a Miller Mark VIII DC welder which were subject to
damage by
the sharp corners of a hole made in the wall of the metal fabrication shop
building.
