Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which are free from recognized hazards that are causing or likely to cause death or serious physical harm to his employees in that employees were exposed to: (a) Big Saw Area: On or about March 17, 2010 and at times prior thereto, employees were exposed to a struck by and crushing hazard while unloading steel from delivery vehicles using unrated under the hook lifting devices. (b) Big Tank Line: On or about March 17, 2010 and at times prior thereto, employees were exposed to a struck by and crushing hazard while unloading steel from delivery vehicles, lifting 46 inch heads, lifting sheets of steel to the shear and feeding sheets of steel into rolls using unrated under the hook lifting devices. (c) Special Fab Area: On or about March 17, 2010 and at times prior thereto, employees were exposed to a struck by and crushing hazard while lifting flanged heads using unrated plate clamps. (d). Big Tank Line: On or about March 17, 2010 and at times prior thereto, employees were exposed to struck by and crushing hazards while operating cranes such as but not limited to the 20 ton and 25 ton overhead bridge cranes used to move sheets of steel, large tanks, sections of pre-fabricated smoke stacks and hoisting personnel while using a hook without a latch across the throat opening. (e). Special Fab Area: On or about March 17, 2010 and at times prior thereto, employees were exposed to struck by and crushing hazards while operating cranes such as but not limited to the two 10 ton overhead bridge cranes used to move sheets of steel, angle iron, and dies for the punch press while using a hook without a latch across the throat opening. Among other methods, one feasible and acceptable method to correct the hazards noted is to follow the requirements in ASME B30.20-1993 BELOW-THE-HOOK LIFTING DEVICES, and ASME B30.2-1996 OVERHEAD AND GANTRY CRANES, which include, among other requirements, the following: 1. Schedule and complete all periodic and annual inspections of under the hook lifting devices. 2. Marking the rated load, as established by a Professional Engineer, on the device where it is visible with a nameplate or other permanent marking with the manufacturers name, serial number, lifter weight and rated load. 3. Latch-equipped hooks shall be used unless the application makes the use of the latch impractical or unnecessary. Abatement Note: Abatement certification and documentation is required for this item.
