Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by hazard: At this inspection site, the employer was using a one-inch rubber hose containing 140 psi compressed air to pump water from the Starboard outboard Number 1 tank on Barge Number 311. On the deck where the two Crowsfoot connections were joined, a straight keeper wire in one ear of the Crowsfoot connection was not twisted which would not prevent the Crowsfoot connection from unintentional parting. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to require that keeper pins/wire be installed in each ear of the Crowsfoot connection and twisted prior to use per the manufacturer's recommendations.
