Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls to lower level: At this location, employees involved in a construction operation were exposed to a fall hazards approximately 22-34 feet in that; 1.only one Crosby clip was used to secure the 5/8 inch wire rope guardrail at each end to the anchorage, 2.the Crosby clips on the 5/8 inch wire ropes used for guard rails were installed backwards with the saddle on the dead end and the "U" bolt on the live end. 3.there were no turnbuckles on the ends of the wire rope guardrails. One feasible means of abatement would be to correctly install 3 Crosby clips at each anchorage point and a turn buckle between the anchorage point and the eye of the 5/8 inch wire rope guardrail.
