IC 22-8-1.1 Section 2:  The employer did not establish and maintain
conditions of work which were
reasonably safe and healthful for employees, and free from recognized
hazards that were causing or likely to
cause death or serious physical harm to employees in that employees were
exposed to crushing hazards:
a) Automotive lift (2 post) serial # 185 GL 10001-  The Tire and Lube
Express Service Technicians were not
trained or otherwise qualified to inspect the Forward lube rack systems,
with a lifting capacity of 12,000
pounds.  Lift systems were used to raise vehicles while the technicians
were underneath changing fluids and
lubricating vehicle components.  The technicians were not conducting
inspections according to the manufacture
or the American Lift Institute (ALI)  instructions as frequently as
required for safe operations.  The
technicians were not properly documenting that inspections were being done.
b) Automotive lift (4 post) serial # 161 IF 8621 -  The Tire and Lube
Express Service Technicians were not
trained or otherwise qualified to inspect the Forward lube rack systems,
with a lifting capacity of 12,000
pounds.  Lift systems were used to raise vehicles while the technicians
were
underneath changing fluids and
lubricating vehicle components.   The technicians were not conducting
inspections according to the
manufacture or the American Lift Institute (ALI)  instructions as
frequently as required for safe operations.
The technicians were not properly documenting that inspections were being
done.
c)  Automotive lift (4 post) serial # 161 DJ 2060 -  The Tire and Lube
Express Service Technicians were not
trained or otherwise qualified to inspect the Forward lube rack systems,
with a lifting capacity of 12,000
pounds.  Lift systems were used to raise vehicles while the technicians
were underneath changing fluids and
lubricating vehicle components.   The technicians were not conducting
inspections according to the
manufacture or the American Lift Institute (ALI)  instructions as
frequently as required for safe operations.
The technicians were not properly documenting that inspections were being
done.
d)  Automotive lift (4 post) serial # 161 FH 0421 -  The Tire and Lube
Express Service Technicians were not
trained or otherwise qualified to inspect the Forward lube rack systems,
with a lifting capacity of 12,000
pounds.  Lift systems were used to raise vehicles while the technicians
were underneath changing fluids and
lubricating vehicle components.   The technicians were not conducting
inspections according to the
manufacture or the American Lift Institute (ALI)  instructions as
frequently as required for safe operations.

e)    Automotive lift (2 post) serial # 110 KH 6278 -  The Tire and Lube
Express Service Technicians were
not trained or otherwise qualified to inspect the Forward lube rack
systems, with a lifting capacity of 9,000
pounds.  Lift systems were used to raise vehicles while the technicians
were underneath changing fluids and
lubricating vehicle components.   The technicians were not conducting
inspections according to the
manufacture or the American Lift Institute (ALI)  instructions as
frequently as required for safe operations.
The technicians were not properly documenting that inspections were being
done.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is:
To develop and implement a suitable qualified inspector, operator training
and maintenance programs in
accordance with the manufacturer recommendations and ANSI/ALI ALOIM 2000
Standard for Automotive
Lifts - Safety Requirements for Operation, Inspection, and Maintenance.
