employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to caught between/struck by and fall hazards and resulting
blunt force trauma from
roof truss collapse:
a) Construction site, St. Gregorios Macankara Orthodox Church - On or about
July 18, 2008 Employees were installing sheathing scissor trusses spanning
64
feet did not install adequate temporary bracing allowing 53 of the 76
trusses to
collapse.
Among other methods, one feasible and acceptable abatement method would be
to follow
recommendations of the Truss Plate Institute and Wood Truss Council of
America BCSI-B2
Summary sheet Guide for Installation and Temporary Bracing, which was
provided with the
trusses by the manufacturer.
These recommendations would include but not be limited to the following:
1. Consult a Professional Engineer for installation and temporary bracing
of
trusses over 60 feet as specified in the first warning paragraph.
2. Install sheathing immediately as stated in the first paragraph regarding
maximum spacing for top chord temporary lateral bracing.
3. Install TCTLR maximum 4 feet O.C. as specified on pg. 1
4. Install temporary bracing throughout, as specified in step 5 thru 8.
5. Pre assemble trusses for installation as documented in BCSI-B11.
Abatement certification required within 10 days after abatement date.  The
certification shall
include a statement that abatement is complete, date and method of
abatement, and states
employees and their representatives were informed of this abatement.
