On or about 8/29/07,
a) The doors that led to the hoistway at the second floor level were not
interlocked. The hoist
could be on a different floor or could be operated while the doors were
open.
Among other methods, one feasible and acceptable abatement method to
correct the hazard is:
to ensure that interlocks and door locking devices are installed and
functioning properly and
follow ASME A 17.3 - 1993 Safety Code for Existing Elevators and
Escalators.
NOTE: The employer is required to submit abatement certification for this
item
in
accordance with 29 CFR 1903.19.
