Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: East Penwell San Andreas Unit, Well #951: On 11/13/07, and times prior thereto, employees were exposed to the hazard of being struck by a failed pressurized flexible hose connected between a truck and well head while treating a well. The hose was not properly secured or restrained to prevent unsafe movement in accordance with American Petroleum Institute Recommended Practice RP54 Paragraph 9.13.3.
