Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: ABCO Fire Protection, Inc., 14202 Highway 87, Lubbock, Texas. For the period of time up to and including December 17, 2007, employees were exposed to caught by hazards while performing work operations such as operating a RIDGID 918 Roll Groover without the foot switch while threading pipes. One employee first digit of the first finger on the left hand was amputated. This condition exposed employees to caught by hazards. Under the RIDGID 918 Roll Groover Operator's Manual, Page 3 states "Using a threading machine without a foot switch increases the risk of serious injury. A foot switch provided better control by letting you shut off the motor by removing your foot. If clothing should become caught in the machine, it will continue to wind up, pulling you into the machine. Because the machine has high torque, the clothing itself can bind around your arm or other body parts with enough force to crush or break bones. Under the RIDGID 918 Roll Groover Operator's Manual. Page 3 states "Do not use this Roll Groover with Power Drive or Threading Machine that does not have a foot switch. Foot switch is a safety device to prevent serious injury. One feasible and acceptable abatement method would be to ensure that employees do not operate the 918 Roll Groover without the foot switch. Note: Abatement certification and supporting documentation are required for this item.
