were causing or likely to cause death or serious physical harm to

were exposed to the hazards of:
(a) On or about Febuary 28, 2008, employees were working in two scissor
lifts and were
standing on the handrails while being tied off to the same handrails
exposing themselves to a 25
foot fall hazard. Among other methods, one feasible and acceptable
abatement method to correct
this hazard is to comply with ANSI/SIA A92.6 1990, paragraph 7.11.1,
Personal Footing.
Personnel shall maintain a firm footing on the platform floor while
working thereon.
