Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of: (a) On or about 02/27/08 at Grey Wolf Drilling Company Rig 82 found near Franklin, TX three malleable cast iron clips were used to make up the Geronimo Line. The center clip was put on backwards. This exposed derrick workers to a fall from elevation hazard. Among other methods, one feasible and acceptable abatement method to correct this hazard is to use forged wire rope clips and install them in accordance with the manufacturer's recommendations. Ref: The Crosby Group Product Warning (ev. 9/16/02) page 11: MALLEABLE Wire Rope Clips, WARNING * Never use malleable clips for critical, heavy duty, or overhead loads, such as lifting slings, support lines, guy lines, towing lines, tie downs, scaffolds, etc. also on this same page (NEVER SADDLE A DEAD HORSE!) OSHA Construction standard 29CFR1926.251(c)(5)(i) When used for eye splices, the U-bolt shall be applied so that the "U" section is in contact with the dad end of the rope.
