Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and/or crushing hazards: (a) On or about 08/23/07 for employees off loading 8-inch diameter steel pipe from the back of a flat bed trailer and exposed to a potential struck-by and/or crushing hazard when the bolt holding the block to the Caterpillar D-4 side boom crane failed and the 8- inch diameter steel pipe fell. Among other methods one feasible and acceptable abatement method to correct this hazard is to follow the requirements set forth in ANSI B30.14-1999 Side Boom Tractors; Chapter 14-2 Inspection, Testing, and Maintenance; Sections 14-2.1 through 14-2.4.3 Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement Certification Letter").tion
