recognized hazards that
were causing or likely to cause death or serious physical harm to an
employee in that the
employee was exposed to a potential fall and/or crushing hazard while
working on a platform
elevated on the forks of a GRADALL rough terrain forklift truck:
a). East Corner of Building.  On or about 08/15/07, an employee was
observed on a
platform elevated on the forks of a GRADALL rough terrain forklift truck
and exposed
to a potential fall and/or crushing hazard.  The platform was not secured
to the lifting
carriage or forks.  The operator did not remain at the controls, and the
platform was not
equipped with controls.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO
CORRECT THIS HAZARD IS TO FOLLOW THE AMERICAN NATIONAL
STANDARDS INSTITUTE ASME B56.6, SAFETY STANDARD FOR ROUGH
TERRAIN FORKLIFT TRUCKS, SECTION 5.15(b) WHICH REQUIRES SECURING
THE PLATFORM AND SECTION 5.15(s) WHICH REQUIRES THE OPERATOR TO
BE AT THE CONTROLS IF THE PLATFORM IS NOT EQUIPPED WITH
CONTROLS.
