On or about December 17, 2008, and at times prior thereto, at the
establishment, located
at 400 West Olrich Street, Rogers, Arkansas, an employee, during the
course of his
duties as Wastewater Operator, was required to access a higher level using
a portable
step ladder which was not rated for the load imposed on the ladder,
exposing the
employee to a fall from elevation.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to never overload a ladder beyond it's duty rating as required by ANSI
A14.5a-1977, SI.4.2
Instructions for Stepladders, Section Proper Climbing and Use, Item (9).
Do not overload.

correct this hazard is
to ensure that every ladder used by employees at the facility were legibly
marked as to warnings
and product information, as required by ANSI A14.5a-1977, S1.3.1.1, S1.5.1
and S1.5.2.
