hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to falls from elevations in the following instances:
On or about April 30, 2008, and at times prior thereto, employees were
exposed to the
hazards of falls from elevations in the following instances:
(a)On or about April 30, 2008, an employee was observed working from the
platform of the Mustang Telehandler, Model RS5, at approximately 28
feet above the level below without benefit of fall protection.
(b)On or about  April 30, 2008, an employee was observed standing on the
midrail of the Mustang Telehandler, Model RS5, at approximately 30 feet
above the level below without benefit of fall protection.
Among other feasible means, acceptable and feasible means of abatement
include (1)
Provide employees working in elevated personnel platforms with personal
fall
protection,
(2) Develop and enforce a work rule requiring employees to maintain firm
footing on the
floor of the platform at all times and prohibiting employees from climbing
on the
guardrail to achieve additional reach or height., (3) Comply with ANSI
B56.6- 2005
Section 5.15.1 Safety Standard for Rough Terrain Forklift Trucks
requirements for
personnel platforms and Subparagraph (u) "personnel are to maintain firm
footing on
platform floor unless secured by a personal fall protection system.. (4)
Provide fall
protection training for employees who are required to access elevations
above 6 feet.
