Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to:
At the worksite, at or near 1000 Bowman Road, Little Rock, Arkansas 72211,
employees
were working from an elevated scaffold platform attached to a rough
terrain powered
industrial truck (forklift). The platform extended approximately sixty
(60) inches past the
width of the truck. Per standards established by The American Society of
Mechanical
Engineers (ASME/ANSI) in part B56.6-1992 (Safety Standard for Rough
Terrain Forklift
Trucks), paragraph 8.25.1(b) (Platforms for Elevating Personnel),
platforms shall not
extend more than 10 inches (250 mm) past the overall width of the truck.
The ASME/ANSI standard states in part, "floor dimensions which shall not
exceed
two
times the load center distance listed on the rough terrain forklift truck
nameplate,
measured parallel to the longitudinal center plane of the truck, nor have
a width greater
than the overall width of the truck [measured across the load bearing
tires plus 10 inches
(250 mm) on either side]."
The work platform was examined and found to have damage to the welds
around the fork
mounting brackets. Additionally, the platform floor structure was warped,
that is, the
ends of the platform were deflected downward from the center where
attached to the
forks of the powered industrial truck. Two employees worked from the ends
of the
platform performing job activities such as but not limited to hanging
insulation and
attaching siding. The employees were working from inside and outside the
platform
without the use of fall protection.
Additionally, the extension of the platform past the width of the truck
created a potential
for a springboarding action, whereby, employees not wearing fall
protection can be
ejected out of the elevated scaffold platform.
These conditions existed on or about May 6, 2008 and times thereto and
exposed
employees to the hazard of falling from an elevation.
Feasible abatement methods include but are not limited to:
1. Providing a platform designed and/or approved by the forklift
manufacturer.
2. Using other means to conduct the job activities.
Abatement Disclaimers:

circumstances.
