On or about March 4, 2008 and days prior, employees were exposed to
struck-by hazards and
crushing hazards due to the employer failing to ensure that the emergency
stop button was not
impeded. A green rubber cap object was positioned over the emergency stop
button thusly
impeding the accessibility of the emergency stop function.
Among other methods, one feasible method is to comply with ANSI/RIA
R15.06-1999, Section
4, Paragraph 4.6.3 Emergency stop design, which states that "each operator
control station,
including pendants, capable of initiating robot motion, shall have a
manually
initiated emergency
stop device. The push button that activates an emergency stop circuit
shall be red in color with
a yellow background, unguarded, and have a palm or mushroom head type".
