Section 6(a) of the Puerto Rico Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or physical harm to employees in that employees were exposed to: Seccion 6(a) de la Ley de Seguridad y Salud en el Trabajo de Puerto Rico: El patrono no proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten causando o que puedan causar muerte o dano fisico a los empleados, por cuanto empleados estaban expuestos a: a)Worksite, project under construction, Bo. El Seco, Mayaguez, P.R. - The screws holder the steel platform was not assure to support the weight and load from the steel platform and it was break and ocasion the fatal accident. Employer was not provided evidence of screws inspection/test or evaluation. Acceptable abatement method to correct, inspection, test and evaluation all parts of scaffolds and platforms by a competent person. a)Sitio de trabajo, proyecto en construccion, Bo. El Seco, Mayaguez, P.R. - Los tornillos que sujetaban los molde/plataformas de acero no fueron asegurados de forma tal que sostuvieran el peso y la carga de los moldes de acero y estos cedieron ocasionando el accidente fatal. Patrono no proporciono evidencia de inspeccion/prueba o evaluacion de los tornillos y las placas a las cuales era agarrado. Entre otros metodos para corregir, piezas de andamios y plataformas de trabajo deben ser inspeccionadas, probadas y/o evaluadas por una persona competente."as,
