Section 6(a) of the Puerto Rico Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or physical harm to employees in that employees were exposed to: Seccion 6(a) de la Ley de Seguridad y Salud en el Trabajo de Puerto Rico: El patrono no proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten causando o que puedan causar muerte o dano fisico a los empleados, por cuanto empleados estaban expuestos a: a)Receive injuries such as: fracture or contusion due to failure in mechanism manual chain hoist, trademark CM, Serie 622, 2 Tons where was used by employees to bring up and down aluminum rolls of 1,555 pounds. The hoist mechanism was not inspected. The hoist mechanism was located in the shutters warehouse, west side. Among others methods, one feasible and acceptable abatement method to correct this hazard is: to performed periodic inspection as defined in ANSI B30.16-1973, Sec. 16-2.2.1.3., pag. 15. a)Recibir lesiones tales como: fractura o contusion debido a falla en el mecanismo de izar de cadenas manual, marca CM, Serie 622, 2 Toneladas donde era usado por empleados para subir y bajar rollos de aluminio de 1,555 libras. El mecanismos de izar no estaba inspeccionado. El mecanismo de izar estaba localizado en el almacen de tormenteras, lado oeste. Entre otros metodos, un metodo aceptable y posible de corregir es: realizar inspecciones periodicas segun se define en ANSI B30.16-1973, Sec. 16-2.2.1.3., pag. 15."
