Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that the critical lift
plan was not followed exposing employees to crushing hazards of the crane:
(a)At the jobsite located at 29th St South of Cleveland over UPRR; on or
about
March 5, 2008; two Lorain L-790 100 Ton Crawler Cranes were used to lift a
beam weighing approximately 87,000 pounds.  The designated person did not
follow the design engineers specifications and did not ensure the proper
positioning, rigging of the load, and the movements to be made.
Among other methods, feasible and acceptable means to correct these
hazards are to use a safer
alternate method such as, but not limited to:
Develop and follow a critical lift form proper filled out for each of the
cranes to be
utilized.
Ensure pre-lift activities are accomplished and recorded.
Ensure the crane operators are familiar with the specifics of the plan.
Ensure the critical lift document is followed with regard to the proper
positioning,
rigging of the load, and the movement of the cranes to be made.
Ensure communication between the two crane operators is established to
communicate
vital information.
Certification, and documentation/evidence of abatement is required.
Certification of abatement is required by 29 CFR Part 1903 and is to be
submitted within ten (10)
calendar days after the abatement date.ment
