Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that workers were exposed to potential paper dust explosion, deflagration, and fire hazards caused by inadequate plant "blow down" procedures: (a)Winder Bay - The following deficiencies were found with plant "blow down" procedures: (1) "Blow down" performed with electric lights and associated wiring and receptacles energized. (2) "Blow down" performed with an energized scissors lift. (3) "Blow down" performed with an energized air compressor. (4) Air hoses for "blow down" were greater than 15 psi gauge pressure. (b)Folded Room - The following deficiencies were found with plant "blow down" procedures: (1) "Blow down" performed with electric lights and associated wiring and receptacles energized. (2) "Blow down" performed with an energized scissors lift. (3) Air hoses for "blow down" were greater than 15 psi gauge pressure. (c)Warehouse - The following deficiencies were found with plant "blow down" procedures: (1) "Blow down" performed with electric lights and associated wiring and receptacles energized. (2) "Blow down" performed with an energized scissors lift. (3) Air hoses used for "blow down" were greater than 15 psi gauge pressure. ABATEMENT NOTE: Among other methods, one feasible and acceptable method to correct this hazard is to ensure workers minimize the generation of dust in accordance with National Fire Protection Association (NFPA) 654 "Standard for the Prevention of Fire and Dust Explosions from the Manufacturing, Processing, and Handling of Combustible Particulate Solids" (2006) as it applies to your operations and equipment, including but not limited to the following: * Perform removal of dust in accordance with Sections 8.2.2.2 and 8.2.3. Certification, and documentation/evidence of abatement is required. Certification of abatement is required by 29 CFR Part 1903 and is to be submitted within ten (10) calendar days after the abatement date.ment
