recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a fall hazard:
On the east side of the warehouse where the Harmony baler is located,
employee(s) are exposed to a fall hazard when using a defective ladder for
access
and egress to the ten-foot deep concrete pit located underneath the
conveyor to
the baler.  The defects on the ladder were, such as, but not limited to,
split and
cracked side rails and missing shoes on both of the bottom rails, causing
the
ladder to wobble and be unstable.
Among other methods, one feasible and acceptable method to correct this
hazard
would be to implement an inspection schedule and procedure for all ladders
and
withdraw any defective ladders from service.
