(a)Warehouse - On or about 1/14/2009 employees were operating a Toyota
forklift,
model # 5FGC15 and Toyota forklift, model # 426FGU25 which did not have
seat belts.
Among other methods, a feasible and acceptable method of correction is to
install a seat
belt retro-fit package and require seat belts to be worn when the forklift
is in operation.
