Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by falling steel storage racks
and/or the components
of the steel storage racks, including any products stored on the racks.
a) Location - Facility
On or about October 17, 2007, employees were exposed to the hazard of
being struck
by collapsing steel storage racks, and the material stored on them, while
loading,
unloading and working under overhead storage.
Among other methods, feasible and acceptable means of abatement would
include all of
the following:
1) Repair or replace all damaged metal storage racks in accordance with the
manufacturer's specifications, and ANSI MH16.2 - 1984(R1996).
2) Develop and implement an effective inspection and maintenance policy
for all
steel storage racks.

were exposed to crushing and fall hazards while operating a vertical
reciprocating conveyor
(freight elevator).
a) Location - Facility
On or about October 17, 2007, employees were operating a vertical
reciprocating
conveyor (freight elevator) with a safety gate that could be opened when
the conveyor
was in use or when the car was at another level exposing employees to fall
hazards
and/or crushing hazards.
Among other methods, feasible and acceptable means of abatement would
include all of
the following:
1) Ensure the vertical reciprocating conveyor is equipped with adequate
gates and
enclosures as prescribed in ASME/ANSI B20.1-2000 section 6.21.2 paragraph
(b)
2) Ensure the vertical reciprocating conveyor meets all applicable
requirements
for design, inspection, testing, maintenance, and operation as prescribed
in
ASME/ANSI B20.1-2000
