Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to potentially hazardous
atmospheres and mechanical hazards when entering a confined space(s):
a/  At the gin, the employer failed to perform a hazard evaluation of the
covered pit below the PHX Press
prior to authorizing employees to enter it for the purpose of performing
cleaning of the space and servicing of
the machinery.
One method of abatement to correct this apparent hazard is to perform the
required hazard evaluation of the
space by a qualified person and document the findings in an entry permit
as required by ANSI Z117.1-2003,
paragraph 3.3.

the hazards of an inadequate
emergency response plan:
a/  At the gin, the employer failed to develop an emergency rescue plan,
specifically for vertical rescue, for
employees who descend down into the covered pit directly below the PHX
Press to perform cleaning and
servicing of the machinery.
One method of abatement to correct this apparent hazard is to develop and
implement an emergency plan with
special emphasis on vertical rescue as required by ANSI Z117.1-2003,
paragraph
14.1.

a/  At the gin, the employer failed to develop specific energy control
procedures for machinery such as PHX
Press which uses a combination of 440-volts, hydraulics, and pneumatics to
operate.
One method of abatement to correct this apparent hazard is to develop
specific energy control procedures in
writing as required by ANSI Z244.1-2003, paragraph 5.3.1.1.

the hazards of being struck-by
punctured compressed gas cylinders:
a/  At the tool room, three oxygen and one acetylene cylinders in storage
were not properly supported to
prevent them from being knocked over.
One method of abatement to correct this apparent hazard is to secure
compressed gas cylinders to prevent them
from falling as required by the Compressed Gas Association Pamphlet,
P-1-1965, section 3.4.4.
