Basement South West Corner:
On or about October 18, 2007
Employees were exposed to the hazards associated with operating
a scissors lift.
a) Grove #73 SM3146E:  Guardrails were improperly installed and
gates and openings were not closed.
b) Grove #74 SM3146E:  A weld was completely separated on the
access ladder.
Among other feasible and reasonable abatement methods to correct
this hazard would be to implement the requirements outlined in the
American National Standards Institute's standard for Self propelled
elevating work platforms, ANSI A92.6 1990, Section 14.1
Operation and 14.2 Restrictions and Requirements. These
requirements include but are not limited to "14.2(5)Ensure that
guardrails are properly installed and gates or openings are closed."
14.1.2(1) Inspection for defects such as cracked welds.
NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION
FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.CE
