Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees are exposed to struck by hazards. Throughout the facility at 2614 Martha St. Omaha, NE 68105 - The company is using overhead and gantry cranes to maneuver loads and equipment throughout the plant. Numerous hooks on these lifting devices are missing the safety latch necessary to ensure the load does not slip off the hook during an unpredictable shift or quick stop of the equipment. Areas which this is evident include, but are not limited to: a) Maintenance Area b) Ladle Lining Area c) DISA Molding Area Among other methods, feasible and acceptable methods of abatement are: 1) Ensure that the hooks on all overhead and gantry cranes are installed and maintained in serviceable condition. 2) Develop and implement an inspection protocol pertaining to the overhead and gantry cranes in the facility to ensure the cranes are kept in serviceable condition at all times. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.
