Section 5(a)(1): 2315 Benson Garden Blvd., Omaha, NE 68134-During cleaning and maintenance activities, employees were required to work on an apartment balcony approximately 20' above a cement walkway. The metal railing detached from the floor causing one employee and one subcontractor to fall to the ground below. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.
