SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. On or about October 19, 2007, employer knew or should have known that employees were exposed to the hazards of potential oxygen deficient atmosphere and entrapment with entry into confined spaces such as, but not limited to water tank. A feasible and useful means of abatement, among others, is to: a) ensure that permit required confined space permit has duration of time, name of attendants, name of entrant(s), periodic monitoring, and rescue procedures listed on permits; b) ensure that attendant is available at all times when a confined space entry is conducted; and c) provide and utilize rescue equipment and personnel, such as, but not limited to, a tripod safety line, body harness, standby person, and any other equipment necessary for confined space entry rescue.
