SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. Employer knew or should have known that employee operating JLG forklift, Model Number 6042, without seat belt worn was exposed to the hazard of being thrown from the forklift. A feasible and useful method, among other methods, to correct this hazard is to ensure operator wears seat belt while operating forklift.

SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. Employer knew or should have known that employees on work platform, elevated by JLG forklift, Serial Number 164026, in excess of 15 feet high, were exposed to the hazard of no way to descend from platform in case of an emergency at the Villa Gardens job site. A feasible and useful method, among other methods, to correct this hazard is to ensure the operator is at controls while employees are elevated.
