A.  Employer knew or should have known that employee operating Lull
forklift, Model 644,
without a seat belt worn was exposed to the hazard of being thrown from
forklift at the
Georgetown Judicial Center job site.  A feasible and useful method, among
others, to correct
this hazard is to ensure operator wears seat belt.
