SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. The employer knew or should have known that six (6) operators working at the 54 inch vulcanizer pot, WSF Industries #CUNY-3448-P, in the drum room were exposed to the hazard of thermal burns due to the improper and lack of maintenance to include, but not limited to, failure to correct the malfunctioning locking ring, check valves, and stagnant water. A feasible and useful means of abatement, amongst others, is to correct deficiencies in equipment that are outside acceptable limits before further use or in a safe and timely manner when necessary means are taken to assure safe operation. B. The employer knew or should have known that six (6) operators working at the 54 inch vulcanizer pot, WSF Industries $CUNY-3448-P, in the drum room were exposed to the hazard of thermal burns due to the lack of development and implementation of standard operating procedures. A feasible and useful method to correct the hazard, among other methods, is to develop and implement standard operating procedures.

Employer knew or should have known that employees working on, around, or near vulcanizer large pot in the drum room were exposed to the hazard of thermal burns from the uncovered steam pipe and non-moving parts on the vulcanizer pots. A feasible and useful means of abatement, among others, is to insulate the vulcanizer pot and steam pipes with heat insulating material.

Employer knew or should have known that employees operating the Komatsu forklift, No. FG35T-8, #141287A, were exposed to the hazard of being thrown from the forklift and crushed due to the forklift not being provided with a seat belt. A feasible and useful means of abatement, among other methods, is to provide and require the use of a seat belt by employees operating the forklift. B. Employer knew or should have known that the employee operating Toyota forklift, #17097, #7FBEV18, not wearing a seat belt was exposed to the hazard of being thrown or crushed by the forklift in the event of a rollover. A feasible and useful means of abatement, among others, is to enforce the use of wearing seat belts while operating the lift.
